Tell us what needs attention
Include the page URL, the specific fact in question, the requested correction, and a source or explanation we can review. Do not email identity documents, insurance files, or sensitive personal information.
Correction requests
Provider identity, coverage, services, contact details, verification state, and publication status can each be corrected independently. A request creates an auditable review; it does not automatically change a public record.
Urgent safety matters
Find Locators is not an emergency service or one-call center. For a suspected utility emergency, stop work, leave the immediate area when appropriate, and use the official emergency and utility contacts for your location.
Privacy and verification disputes
State the record involved and the type of request. Sensitive evidence will be collected only through a private authenticated workflow after identity and authority are established.
